Five Steps to Access Your Carisk Behavioral Health Benefits

Faculty, staff, and their dependents who participate in the UM/Aetna health plan also receive mental and behavioral health benefits through Carisk Behavioral Health (formerly Concordia Behavioral Health), including comprehensive inpatient and outpatient care for physical and psychosocial needs, substance abuse, and pain management. All care is provided with the fullest degree of confidentiality.

1. Know your Aetna plan. Aetna 1 and Aetna 2 participants have access to in-network coverage only. HRA participants have access to in-network and out-of-network benefits.

2. Search for a provider before contacting Carisk. Visit www.concordiabh.com and select the Find A Provider tab on the top-right of the page, then click the Commercial Directory.

3. Contact Carisk for prior authorization to visit your selected provider at 1-800-294-8642, option 2, Monday to Friday, 8 a.m. to 5 p.m.

4. Review copays and coinsurance to understand your out-of-pocket cost.

5. Call a clinician. Clinicians are on call 24/7 at 1-800-294-8642, option 2.
